We are using Google Forms for registration. If you do not have access to a computer you may use the hard copies below.

Mini Moose Google Registration HERE!

A $20 per child or $35 family max, yearly, non-refundable enrollment fee needs to be included with the forms to complete your registration.
This fee covers yearly consumable items such as; paper, crayons, glue, snack etc.
